Release of Information

In order for our psychologists and psychiatrist to communicate and collaborate with your pediatrician, primary care doctor/specialists, therapists, and schools, we will ask you to sign a release of information at your first appointment. At times, our professional office administrators will also need to communicate with outside doctors’ offices or school personnel and will necessarily be included in this release of information. All our employees strictly adhere to all HIPAA rules and regulations to ensure your complete privacy.

Missed Appointment Policy

New clients

Due to our long waiting list of clients needing to be seen, if you miss your confirmed scheduled appointment for your initial clinical interview, it is possible you may not be able to reschedule with us and will unfortunately be referred to another facility.


Established clients

We understand that sometimes emergencies and illnesses do happen and you will be unable to attend your appointment even though you confirmed the day before. However, after your second late cancellation or no-show appointment that occurs over a short period of time, (within six weeks) your chart will be reviewed by your doctor and a determination will be made as to whether we regretfully must refer you to another facility.


All new and established clients:

If we are unable to reach you by 3pm the day before your appointment because your phone numbers have changed or are no longer in service or we are only able to leave a voice mail message, your appointment may necessarily be filled with one of our waiting families. Consequently, if your phone numbers change or are no longer in service, you must call us by 3pm the day before your appointment to avoid losing your appointment.


Testing Appointment Policy

All of our psychologists have a long list of patients scheduled to be tested. Because these testing appointments are typically 3 to 4 hours in length and are so important to the diagnosis and treatment of our patients, we do require ‘live’ confirmation from you the day before your appointment. If we are unable to reach you by 3pm the day before your testing appointment because your phone numbers have changed or are no longer in service, or we are only able to leave a voice mail message, your testing appointment may necessarily be filled with one of our waiting families. Consequently, if your phone numbers change or are no longer in service, you must call us by 3pm the day before your appointment to avoid losing your testing appointment.
